• Diagnosis as determined by the technician — which may differ from the described fault

• Work authorised by the customer (obtain verbal or written authorisation before proceeding)

• Estimated completion date and estimated cost (where applicable)

Work Completed

• Every service action performed, in sufficient detail to be reconstructed from the record

• Every part used: part name, part number, quantity, unit cost, line total

• Labour time: hours spent by each technician

• Any recommendations for future work not completed this visit

Financial Summary

• Parts total

• Labour total (hours × labour rate)

• Total job cost

• Payment method and receipt reference

Sign-Off

• Technician name and signature

• Workshop supervisor sign-off

• Customer signature on collection confirming work received and payment made
